CareVitality can provide transitional care management services to help your employees and students transition from a hospital, skilled nursing facility, rehab facility or other such type of facility to finish recovery in their homes and avoid readmissions.
We work closely with your employees and students to make sure they have 24/7 access to care, meals, transportation and caregiver services when needed with a speedy recovery to return back to work and avoid additional medical expenses.
