Patient Engagement

We develop Comprehensive Care Plans based on the chronic conditions of the patient and have the health care provider review and sign off on it.

We Facilitate Care Transitions to a Qualified Health Center

We communicate with GCM patients, their GCM providers and other providers responsible for the patient’s care, we manage their care transitions between other setting and other providers and ensure that information becomes part of their patient chart.

We Manage & Monitor Patient Care

As the chronic condition of the patient changes, we revise their care plan to provide an accurate continuum of care, each revision must be signed off on by the GCM billable provider.

We Provide Auditable Reports

At the the end of each month CareVitality provides your staff with an auditable report of the work completed by our Clinical Care Team so you can easily bill CPT Code G0511 for your GCM patients. Utilizing our GCM platform we can send all completed work directly into your billing system to be processed.

GCM Documentation in Your EHR

Our GCM documentation is saved within the patient’s electronic medical record in your EHR for continued access and review by their medical provider or clinical staff needing the information.

Payment Is Simple.

CareVitality is Paid on Performance and Completely at Risk. We are paid a flat fee at the end of the following month in which services are rendered which at this time you should be paid by Medicare and the rest of the general care management services billing is yours to keep as profit.

CareVitality’s Compliance Team reviews the GCM monthly report prior to being sent for Medicare payment to ensure not only the time element but the full scope of service is met for the CPT Code G0511.

What Is the General Care Management Service CPT Code?

CPT Code G0511

Chronic care management services meeting the full scope of services for a minimum of 20 minutes of clinical staff time directed by a physician or other qualified health care professional, per calendar month, with the following required elements:

Patient must have multiple (two or more) chronic conditions expected to last at least 12 months, or until the death of the patient to qualify for service. Additionally, all chronic conditions need to be reviewed along with the full scope of services prior to billing HCPCS G0511 for this service in any given month.
Chronic conditions place the patient at significant risk of death, acute exacerbation/decompensation, or functional decline.
Comprehensive care plan established, implemented, revised, or monitored.
